Poll: Will You Follow the New Guidelines for Cervical Cancer Screening?

April 17, 2012

New screening guidelines for the prevention and early detection of cervical cancer are now largely based on the patient’s age, and testing for HPV DNA has now been incorporated into the screening process. The new recommendations are:

No screening before the age of 21.
Pap every 3 years from age 21 – 29, but no HPV screening.
Pap and HPV testing every 5 years from age 30 – 65.

Benefits to these new guidelines include: more time spent discussing BSE, referral for mammography, healthy diet and exercise, risky behaviors, family planning, menopausal transition, osteoporosis prevention, colorectal cancer screening, and evaluation of continence and pelvic floor functioning.

If you follow the new guidelines, will you encourage your patients to come in every year to discuss other health issues?  We want to know, add your opinion to the comment box!

I want to know why screening not start befor the age of 21

by Jenn Klein | May 02, 2012 1:46 PM EDT

I think updating the cervical cancer guidelines is a great idea. Working in the med field, I strongly believe that TOO much is placed on female exams. I am a mother of a 17 yr old daughter who is happy and healthy. She sees our GP yearly for a physical and he has never mentioned anything about a cervical screening.

Any provider who disagrees with the new guidelines is only disagreeing to keep their pockets full of patient/insurance premiums.

Robin- How much does your office get reimbursed for a pelvic exam and paps? If your patients only had a pap's every 3 years, how much would your office lose?

As i said, I have worked in this field for many years, I have never once agreed with the guidelines for cervical screenings. I believe Drs offices use fear to keep females trained to come in yearly and get an exam, its pathetic.

I feel the new guidelines will miss finding significant disease in a subset of women in their early twenties who have been sexually active since early teens... while it is still treatable and they have access to care in student health centers. Also, if their first pap smear happens to be a false negative (as 30% are), you miss the opportunity to find it earlier by waiting 3 years to the next screen.

It is premature for us to adopt the policies of the European countries that have almost 100% compliance to the HPV vaccine and universal health care for all. About 40 % of twenty somethings are uninsured in this country.

I am quite alarmed by the guidelines, and feel my patient population will be as well.
